The Human Flourishing Profile™ is an individual, strengths-based process that brings together a young person's circumstances, relationships, participation, interests and aspirations alongside the conditions surrounding their wellbeing and opportunity. It is designed for children and young people with disability or complex needs, and the families, carers, schools and services supporting them.

The Profile is non-diagnostic and is designed to complement — not replace — clinical, therapeutic, functional, behavioural, risk, statutory or NDIS planning processes.

Coordinated Planning Meeting

A facilitated conversation bringing together families, schools, services and support networks to develop a shared understanding and coordinate practical next steps
