Information we collect

  • We collect personal information you choose to provide through our enquiry and Expression of Interest forms — such as your name, role, email address, phone number, preferred contact method, general location, and any comments you include.
  • For Expression of Interest registrations, we may also collect general planning information such as a participant’s age range, town or region, NDIS participation status and preferred future service format.
  • We ask that you do not submit incident reports, diagnoses, medical information, detailed behavioural histories, restrictive-practice details, documents or other sensitive clinical information through our online forms.

How we use your information

  • To respond to your enquiry and help you work out an appropriate next step.
  • To record your interest in future services, understand regional demand and plan potential service capacity.
  • To contact you about future availability or service-launch updates, where you have asked us to do so.
  • To meet applicable legal, professional and record-keeping obligations.

Storage and security

  • Your information is stored securely in access-controlled systems.
  • It is accessed only by Dr Carol Schultz, or a person authorised to assist with the administration of The Human Flourishing Practice™.
  • We take reasonable steps to protect your information from misuse, loss, unauthorised access, modification or disclosure.

Sharing your information

  • We will not sell, rent or trade your information.
  • We will not share your information unless you consent, disclosure is required or authorised by law, or it is necessary to lessen or prevent a serious threat to life, health or safety.

Access, correction and withdrawal

  • You may request access to, correction of, or withdrawal of your information at any time.
